Without paying due attention to a patient’s medical history and asking any questions to augment it where necessary, you can’t be sure of eliminating potential problems...

A dentist decided to harvest bone from the hip to augment a ridge but failed to notice that the patient had undergone a previous operation on the same hip. The patient had not volunteered this key information and it was not picked up in the medical history. The patient subsequently suffered catastrophic damage to the hip and could no longer walk properly. 

Learning point:
Direct questioning to augment the patient’s responses to a medical history questionnaire should vary according to the dental procedure proposed.
