*1 Part-time involves those who have a contract with Hadassah Dental. All other part-timers pay the full rate.
*2 A dentist who is involved in either the placement of a prosthetic element on an implant or the placement of an implant into the mandible or maxilla, is required to pay one of the implant-related grades.
*3 Full general practice including extended procedures carried out in the lip and peri-oral area including the naso-labial folds, glabella and 'crows feet' (around the eyes), forehead and cheeks, but excluding the neck. These extended procedures are defined as Botox (whether administered for cosmetic or therapeutic reasons), any kind of collagen replacement therapy, fillers (eg Restylane, Perlane), demabrasion and/or wrinkle reduction treatments not involving the use of lasers.
